- Diagnosis and medical needs should be documented by the treating physician, nurse, or patient advocate
- Proof that the patient does not have prescription drug coverage and does not qualify for private or public health insurance
Once the application is complete, it will be reviewed by health professionals at Moss Clinic. They will then determine if you are eligible for prescription assistance services.
